You have the right to request access to your medical record and other health information we maintain about you. This may include electronic or paper copies. We will provide access within 30 days of your request. A reasonable, cost-based fee may apply.
If you believe that information in your medical record is incomplete or incorrect, you may request a correction or amendment. If we deny your request, we will provide a written explanation.
You may request that we communicate with you in a specific way or at a specific location, such as using a certain phone number or mailing address. We will accommodate reasonable requests.

You may request a list of certain disclosures of your PHI made by Honor Behavioral Health within the past six years.

If you have a legal guardian, healthcare proxy, or a person designated under medical power of attorney, that individual may exercise your rights on your behalf.
If you believe your privacy rights have been violated, you may file a complaint with Honor Behavioral Health or with the U.S. Department of Health and Human Services (HHS). We will not retaliate against you for filing a complaint.

We use and share your PHI to provide, coordinate, and manage your care. This may include sharing information with therapists, psychiatrists, physicians, or other members of your treatment team.
We may use and share your PHI to bill and receive payment from health plans or other payers for services provided.
We use your PHI to support our operations, including quality improvement, staff training, audits, compliance activities, and accreditation.
We may use or disclose PHI to report suspected abuse or neglect, prevent or reduce a serious threat to health or safety, and support public health reporting requirements.
We may use or disclose limited PHI for research purposes in accordance with applicable laws and regulations.
We may disclose PHI when required by state or federal law.
PHI may be disclosed to medical examiners or coroners as authorized by law.
We may release PHI as necessary to comply with workers’ compensation programs, law enforcement requests, health oversight activities, and national security or intelligence activities.

Honor Behavioral Health is legally required to maintain the privacy and security of your PHI, notify you promptly if a breach occurs that may compromise your information, follow the privacy practices described in this Notice, and limit use or disclosure of PHI to what is permitted by this Notice or authorized by you in writing. You may revoke written authorization at any time.
Honor Behavioral Health may update this Notice periodically. Any revised Notice will apply to all PHI we maintain and will be posted on our website and available upon request.
